Join MGA as we support the process of developing a caregiving assessment through MGA member Carmel of CMK Home Care. The process will be broken down into three segments with time to offer feedback after each one. The tool is designed to help professionals in the field conduct an effective caregiving assessment. Professionals could be COA outreach workers, Geriatric Care Managers, all home care agency case managers, and more.
Caregiving Assessment helps solve caregiving problems for older people and their families and/or supports. The four major areas of coverage will include:
- Client Need Assessment
- Plan of Care Development for the Older Adult
- Family Caregiver Support, Role, Management Assessment
- Integration of the Care Plan
